dc.description.abstractTIIVISTELMÄ Jonna Paukkonen (2014). Liikkumisen apuvälineen käyttö lonkkamurtuman jälkeen. Terveystieteiden laitos, Jyväskylän yliopisto, Gerontologian ja kansanterveyden pro gradututkielma, 43 s. Tutkimuksen tarkoituksena on selvittää mitkä tekijät ovat yhteydessä liikkumisen apuvälineen käyttöön keskimäärin 70 päivää lonkkamurtuman jälkeen aiemmin ilman apuvälineitä liikkuvilla henkilöillä.Tutkielma on osa The Promoting Mobility after Hip Fracture (ProMo) tutkimusta ja siinä hyödynnetään ProMo – tutkimuksen alkumittausaineistoa (n=81). Tutkittavat rekrytoitiin vuosina 2008 -2010 Keski-Suomen keskussairaalassa lonkkamurtuman vuoksi leikatuista yli 60-vuotiaistahenkilöistä. Tähän tutkielmaan valittiin ne tutkittavat, jotka eivät käyttäneet liikkumisen apuvälineitä ennen murtumaa (n=50). Tutkittavien keski-ikä oli 77v., naisia heistä oli 74 %.Liikkumisen apuvälineiden käyttö ennen murtumaa ja keskimäärin 70 päivää murtuman jälkeen (ProMo – tutkimuksen alkumittaukset) selvitettiin kyselylomakkeella. Tutkielman analyysejä varten tutkittavat jaettiin kolmeen ryhmään alkumittauksissa raportoidun apuvälineiden käytön suhteen; 1)ilman apuvälinettä, 2)kävelykepin-, kyynär-, ja kainalosauvojenkäyttäjät (keppiryhmä) ja 3)rollaattorin käyttäjät.Toiminnallinen tasapaino mitattiin Berg’s Balance Scale (BBS) ja koettu tasapainon varmuus The Activities-specific Balance Confidence (ABC) – mittarilla. Polven ojentajien isometrinen voima mitattiin dynamometrillä ja molempien alaraajojen ojentajien voimantuottoteho Power Riglaitteella. Tutkittavien yhteisnäöntarkkuus testattiin Landoltin näkötaulun avulla. Eri apuvälineiden käyttäjäryhmiä verrattiin ristiintaulukoinnin, khiin neliötestin, yksisuuntaisen varianssianalyysin,Kruskall-Wallis- ja Bonferroni post hoc–menetelmin. Rollaattorin käyttöön yhteydessä olevia tekijöitä selvitettiin logistisella regressioanalyysillä (näkö, leikkaustapa, BBS, ABC ja lihasvoima).Regressioanalyysiä varten Ilman apuvälinettä- ja keppiryhmä yhdistettiin. Heikko näkö, heikko toiminnallinen tasapaino, heikko tasapainon varmuus ja heikko lihasvoima olivat tilastollisesti merkitsevästi yhteydessä rollaattorin käyttöön lonkkamurtumasta toipuvilla iäkkäillä henkilöillä, jotka ennen lonkkamurtumaa olivat liikkuneet ilman apuvälinettä. Osteosynteesi leikkaustapana lisäsi rollaattorin käytön riskiä yli viisinkertaisesti (OR 5.464, 95 % LV 1.63-18.36). Monimuuttujamallissa tasapainon varmuus (ABC) säilyi edelleen tilastollisesti merkitsevänä rollaattorin käyttöä selittävänä tekijänä (p=.013).Heikentynyt tasapaino, heikentynyt lihasvoima, heikentynyt näkö ja lonkkamurtuman leikkaustapa ovat yhteydessä rollaattorin käyttöön noin kaksi kuukautta lonkkamurtuman jälkeen iäkkäillä henkilöillä, jotka ennen murtumaa eivät olleet käyttäneet liikkumisen apuvälineitä. Merkitsevin yhteys on heikentyneellä tasapainon varmuudella. Heikentynyt tasapainon varmuus kuvaa osittain kaatumisen pelkoa. Tasapainon varmuus tulee huomioida kuntoutuksessa ja suunnata jatkotutkimukset tasapainon varmuuteen vaikuttaviin tekijöihin.fi

Department of Health Sciences, University of Jyväskylä, Master’s Thesis of Gerontology and Public Health, 43 pp. The purpose of this study is to investigate which factors are associated with use of walking aids on average 70 days after hip fracture in people who doesn’t have use walking aid before the fracture.The study is a part of The Promoting Mobility after Hip Fracture (ProMo) research project. The ProMo investigated effectiveness of rehabilitation intervention in over 60-year –old hip fracture patients (n=81). They all were operated in the Central Finland Central Hospital. The use of walking aids before and on average 70 days after hip fracture was inquired by a questionnaire. For this analysis only those who haven’t use walking aid before the hip fracture was chosen (n=50). The participants average age was 77 years and 74 % were women. Three study groups were formed according to the use of walking aids post hip fracture; 1) without walking aid,2) cane, forearm crutch or crutch 3) walker. Functional balance was measured by Berg’s Balance Scale (BBS) and the balance confidence by Activities-specific Balance Confidence (ABC). The maximal voluntary isometric knee extension force was assessed with the use of an adjustable dynamometer chair and the leg extension power was measured also in both lower limbs with the use of the Power rig. Differences between the groups were assessed by cross-tabulation and χ2-test, variance analysis,Kruskall-Wallis and Bonferroni post hoc tests. Logistic regression analysis was used to investigate the factors which are associated with the use of a walker. For the logistic regression analysis the three study group was connected in two study group; 1) without walking aid and cane, forearm crutch or crutch and 2) walker. The vision, type of the surgery treatment, BBS, ABC and the muscular strength were investigated one at the time and then with the multivariate analysis.Poor vision, poor balance, poor balance confidence and poor muscle strength were significantly associated with the use of walker on average 70 day after hip fracture. The osteosynthesis as a type of the surgical treatment increased the risk of using walker fivefold (OR 5.464, 95 % LV 1.63-18.36). The model where all the independent factors were together, the balance confidence remained still significant factor (p=.013). The attenuated balance, attenuated muscle strength, poor vision and the type of the surgery treatment are associated with the use of walker on average two months after the hip fracture, with people who doesn’t have used walking aid before the hip fracture. The most significant association has attenuated balance confidence. The balance confidence illustrates partly the fear of falling. The balance confidence has to take into consideration in the rehabilitation and more research is needed in this area. Key words: mobility, walking aid, balance confidenceen
