Rannekanavaoireyhtymä : leikkaus vai konservatiivinen hoito?

Abstract

Rannekanavaoireyhtymä on tavallisin yläraajan leikkaustoimenpiteen aihe. Retrospektiivisessa poikkileikkaustutkimuksessa alle puolet 230 potilaasta oli ennen leikkausta saanut muuta hoitoa kuin tulehduskipulääkkeitä. Noin puolella tutkittavista esiintyi haittaavia käsioireita, kun leikkauksesta oli kulunut hieman yli vuosi. Hoitotulokset eivät yksiselitteisesti puolla leikkaukseen suoraan ohjaavaa hoitokäytäntöä. Tutkimusten mukaan osa potilaista hyötyy injektiohoidosta, lastahoidosta ja käden aktiivisesta mobilisaatiosta. Leikkaushoitoa voidaan suositella, kun oireena on pareesi tai selvä tuntopuutos, ja niille, joilla konservatiivinen hoito ei tuo apua kipu- ja puutumisoireisiin.

Carpal tunnel syndrome (CTS) is the most common indication to operate on the upper limb. This retrospective study was carried out to determine the number of CTS patients who had received conservative treatment before the operation and to assess the frequency of postoperative symptoms. A questionnaire was mailed to 273 operated CTS patients (mean age 48 years) and was returned by 86 %. It was found that less than half of the patients had received other conservative treatment than anti-inflammatory painkillers before the operation. Postoperatively approximately half of the patients still had impaired hand function. The occurance of hand symptoms was found to be higher the more time had passed since the operation. According to the evidence available a selected group of patients may benefit from steroid injections, splinting, and stretching exercises of the hand. Thus conservative care of the patients with CTS should be emphasized as a logical first step. Surgery is recommended in cases with paresthesia or sensory deficiency or if conservative treatment does not alleviate the symptoms.
