dc.description.abstractTutkimuksemme tarkoituksena oli tarkastella yhden uhmakkuushäiriö- ja Aspergerin oireyhtymän diagnoosin saaneen lapsen ja hänen perheensä hyvinvoinnissa tapahtunutta muutosta perheterapiaprosessin aikana. Aihetta on olennaista tutkia, jotta kliinikoiden ymmärrys lisääntyisi psykoterapian aikana tapahtuneen muutoksen moniulotteisuudesta sekä siitä, mitkä terapian sisäiset ja ulkoiset tekijät saattavat vaikuttaa siihen. Tutkimuksessamme käytetty Hermeneutic Single Case Efficacy Design (HSCED)-tutkimusmenetelmä hyödyntää kliinikoiden jokapäiväisessä työssään käyttämiä menetelmiä, mikä kaventaa tutkimuksen ja käytännön välistä kuilua. HSCED- tutkimusmenetelmän mukaisesti tarkastelimme, onko tutkittavan lapsen ja hänen perheensä hyvinvoinnissa tapahtunut muutosta perheterapiajakson aikana sekä sitä, onko tapahtunut muutos perheterapian aikaansaamaa. Lisäksi selvitimme, mitkä spesifit terapian sisäiset ja ulkoiset muuttujat ovat voineet vaikuttaa tapahtuneeseen muutokseen. Aineistomme koostui yhden perheen kotona ja kodin ulkopuolisissa tiloissa toteutetuista perheterapiaistuntojen videotallenteista, taustatiedoista sekä lomakeaineistosta. Tutkimusprojekti toteutettiin yhteistyössä Jyväskylän yliopiston Psykoterapian opetus- ja tutkimusklinikan ja Kuopion yliopistollisen sairaalan lastenpsykiatrian poliklinikan kanssa. Tulokset osoittavat, että tutkittavan lapsen ja hänen perheensä hyvinvoinnissa tapahtui positiivista muutosta perheterapiajakson aikana, mikä on erittäin todennäköisesti perheterapian aikaansaamaa. Terapian sisäisiä, muutokseen mahdollisesti vaikuttaneita tekijöitä olivat perheterapian toteuttaminen joustavasti perheen kotona ja kodin ulkopuolisissa tiloissa sekä videotallenteiden läpikäynti yhdessä terapeuttien ja vanhempien kesken. Terapian ulkopuolisia tekijöitä olivat perheen kuopuksen syntymä, äidin oma psykologikontakti sekä isän työhön sitoutuminen. Tutkimustuloksemme demonstroivat psykoterapian aikana tapahtuneen muutoksen moniulotteisuutta ja auttavat kliinikoita hahmottamaan usein vaikeasti tulkittavissa olevaa muutosta sekä siihen vaikuttaneita tekijöitä. Tuloksemme tarjoavat uusia näkökulmia ja sovellusmahdollisuuksia perheiden kanssa työskenteleville psykologeille, perheterapeuteille ja muille ammattihenkilöille. dc.description.abstractThe aim of this study was to investigate the change occurred within the well-being of a child and his family during their family therapy process. The child had been diagnosed with Oppositional Defiant Disorder and Asperger syndrome. This research subject is relevant due to clinicians’ lacking understanding of the multidimensional nature of change caused by psychotherapy as well of the specific within-therapy factors and nontherapy factors that may affect to the change process. Hermeneutic Single Case Efficacy Design (HSCED) is a research method utilizing methods for data collection that are often previously familiar to clinicians, which provides an opportunity to bridge the research-practice gap. In this study, we addressed the standard HSCED research questions of whether a change has occurred within the well-being of the child and his family during their family therapy process and whether this possible change was due to the family therapy, as well as which specific within-therapy factors and nontherapy factors may have affected to this particular change process. In this study, the collection of data consisted of video tapes of the family therapy sessions that were conducted both at the family’s home and outside of the family’s house, background information of the family members and the therapists, and quantitative measurements. This research project was implemented in cooperation between the Psychotherapy Training and Research Centre of University of Jyväskylä and Kuopio University Hospital Child Psychiatry clinic. The results of this study indicated that there had been a significant change occurring within the well-being of the investigated child and his family during the family therapy process, which was most likely due to the therapy. The within-therapy factors that were possibly affecting the process of change consisted of the adaptable implementation of the therapy both inside and outside of the home of the family, as well as watching the video tapes of the therapy sessions together with the therapists. The nontherapy factors that were possibly affecting the process of change consisted of the birth of the youngest child of the family, the separate psychological treatment of the mother and the father’s commitment towards his career. The outcomes of this study demonstrated the multidimensional nature of the change processes occurred within the framework of psychotherapy and the factors that may affect to it. Overall, this study provides new perspectives as well as suggestions for further applications for psychologists, family therapists and other professionals of mental health.
