Multimodal vibroacoustic music therapy for functional neurological disorder : development of a clinical model
Toiminnallinen neurologinen häiriö (FND) on häiritsevä tila, jossa hermoston toiminnan häiriintyminen aiheuttaa potilaalle erilaisia motorisia, sensorisia ja/tai kognitiivisia oireita. FND on laajalti väärinymmärretty, mutta yleinen häiriö, jonka esiintyvyys on sama, kuin MS- tai Parkinsonin taudilla. Huolimatta viimeaikaisesta edistyksestä positiivisten diagnostisten tutkimusten ja arvioiden kehityksessä, FND aiheuttaa kliinikoille hankaluuksia hoidon suunnittelussa häiriön luontaisen heterogeenisyyden vuoksi. Vastauksena tähän tuore FND-tutkimus suosittaa monialaisia, multimodaalisia ja integroivia hoitosuunnitelmamalleja yksilöllisen hoidon takaamiseksi. Sekä vibroakustinen hoito että aktiivisen musiikkiterapian luontainen joustavuus ja multimodaaliset mahdollisuudet nähtiin tässä väitöstutkimuksessa ja tämän kliinisen kohderyhmän hoidon näkökulmasta keskeisinä.
Tämä väitöskirja kuvaa multimodaalista vibroakustista musiikkiterapiaa hyödyntävän FND:n hoidon kliinisen protokollan jalostamista ja arviointia. Työn laajana tavoitteena on rakentaa kattavampaa ymmärrystä FND:stä tarvittavan tietopohjan rakentamiseksi tulevia laajempia vaikuttavuustutkimuksia varten. Artikkeli I esittelee yksittäisen monimenetelmäisen pilottitapaustutkimuksen, jossa alustavaa kliinistä protokollaa on käytetty terapiaprosessissa potilaalle, jolla on FND ja liitännäisiä dissosiatiivisia oireita. Tämän pilottitutkimuksen tulokset mahdollistivat kliinisen protokollan jatkojalostuksen. Tuloksena syntynyt, hiottu MTFUND (Music Therapy for Functional Neurological Disorder) -protokolla on kuvattu yksityiskohtaisesti artikkelin II ensimmäisessä osassa. Artikkelin toinen osa tarkastelee yksilöiden kokemuksia protokollasta sekä asiakkaan että terapeutin näkökulmista. Lopuksi artikkeli III esittelee 10 yksilöterapiaprosessin monimenetelmäisen tapaustutkimussarjan. Näiden kolmen tutkimuksen löydökset tarjoavat yhdessä arvokasta tietoa useista näkökulmista luoden kattavan ymmärryksen aiheesta ja tarjoten mahdollisuuksia jatkotutkimukselle.

Functional Neurological Disorder (FND) is a distressing condition in which patients experience motor, sensory, and/or cognitive symptoms as a result of problematic functioning of the nervous system. Though widely misunderstood, it is common, with incidence rates similar to multiple sclerosis and Parkinson’s disease. Despite recent advances in development of positive diagnostic examinations and assessments, FND poses difficulties for clinicians in terms of treatment planning, due to the disorder’s inherent heterogeneity. To answer this variability, contemporary FND literature has recommended multidisciplinary, multimodal, and integrated models of treatment planning to promote individualised care. As relevant to this dissertation, the inherent flexibility and multimodal potentials of both vibroacoustic therapy and active music therapy methods were considered as especially relevant for this clinical group.
This dissertation follows the refinement and evaluation of a clinical protocol for FND, utilising multimodal vibroacoustic music therapy. The broad aim of the work is to form a more comprehensive understanding of FND, in order to gain the necessary basis of knowledge to inform future larger-scale effectiveness studies. Article I presents a single, mixed-methods pilot case study in which the preliminary clinical protocol is used in the therapy process of a patient with FND and related dissociative symptoms. The results of this pilot study allowed for further refinement of the clinical protocol. The resultant refined MTFUND (Music Therapy for Functional Neurological Disorder) protocol is detailed in the first part of Article II, with the second part of the article looking to the experiences of individuals within the protocol from both the patient and therapist perspectives. Finally, Article III presents a mixed methods case series of 10 individual therapy processes. Together, the findings from the three studies provide valuable information from multiple perspectives to contribute a comprehensive level of understanding, along with implications for further investigation.
