Ilmiöt: Vanhemmuustaidot, vanhemman ja lapsen välinen suhde ja vuorovaikutus
Johdanto: Vanhemman ja lapsen välisellä suhteella ja vanhempien vanhemmuustaidoilla on keskeinen merkitys lapsen hyvinvoinnille ja kehitykselle. ICDP – Kannustava vuorovaikutus -ohjelma (International Child Development Programme) on kaikenikäisten lasten ja nuorten vanhemmille ja lasten hoivaajille suunnattu useimmiten ryhmämuotoisesti toteutettava ohjelma, jonka tarkoituksena on vahvistaa hyvää, positiivista vuorovaikutusta ja hoivanantajan sensitiivisyyttä ja kykyä kohdata lasta kunnioittaen sekä vahvistaa vanhemman itseluottamusta vanhempana, vaikuttaa myönteisesti vanhemman käsitykseen lapsesta ja tukea lapsen kuulluksi tulemista.
Tutkimuskysymys: Onko Kannustava vuorovaikutus (ICDP) -ohjelmalla vaikuttavuutta 0–18-vuotiaiden lasten vanhempien vanhemmuustaitoihin ja vanhempi–lapsisuhteeseen suomalaisessa kontekstissa?
Aineisto ja menetelmät: Kirjallisuushaku tehtiin kartoittaville kirjallisuuskatsauksille tyypillisellä Population, Concept, Context (PCC) -hakustrategialla kohdistuen kysymykseen: Mitä tutkimusta Kannustava vuorovaikutus (ICDP, International Child Development Programme) -ohjelmasta on tehty? Hakusanoina käytettiin: P = 0–18-vuotiaiden lasten ja nuorten vanhemmat, C = International Child Development Programme (Kannustava vuorovaikutus -ohjelma), C = Vanhemmuustaidot ja vanhempi-lapsisuhde. Ensisijaisena kiinnostuksen kohteena haussa olivat menetelmään liittyvät vaikuttavuustutkimukset, mutta mukaan otettiin kuitenkin myös muilla asetelmilla tehtyjä tutkimuksia. Systemaattinen haku kohdistui tietokantoihin MEDLINE, APA PsycInfo, CINAHL, ERIC, Web of Science ja Scopus, ja sitä täydennettiin manuaalisella haulla.
Tulokset: Kirjallisuushaun perusteella mukaan otettiin kahdeksan tutkimusta. Näistä kuusi oli pohjoismaisia vaikuttavuustutkimuksia, joihin menetelmän vaikuttavuuden arviointi perustui. Lisäksi mukana oli kaksi pohjoismaista implementointitutkimusta. Ohjelma ei perustu selkeään manuaaliin, joten sen kuvausta arvioitiin hyödyntäen eri lähteitä. Kohderyhmän, toteutuksen, vasteiden sekä ohjaajille suunnatun oppaan osalta ohjelman kuvaus arvioitiin nimekkeellä “kuvattu mutta ei riittävästi”. Ohjelman kohdeilmiö, tavoitteet, keinot, keskeiset elementit ja joustavuus, toteuttajat sekä ohjelman saajille suunnatut materiaalit oli sen sijaan kuvattu riittävästi. Ohjelma nojautuu moniin teorioihin, joiden kuvaus arvioitiin riittäväksi. Ohjelman vaikutusmekanismit perustuvat yleisesti hyväksyttyihin ihmisten toimintaa selittäviin teorioihin. Tutkimuksissa ohjelmalla havaittiin olevan myönteisiä vaikutuksia osaan tarkastelluista vasteista. Ohjelmalla havaittiin olevan tilastollisesti merkitseviä myönteisiä vaikutuksia Brekken ja kollegojen (2023) artikkelissa luokiteltuihin päävasteisiin vanhempien minäpystyvyyteen ja emotionaaliseen sensitiivisyyteen – tunteita ohjaavan käyttäytymisen osalta – sekä heti ICDP-ohjelmaan osallistumisen jälkeen että seurannassa neljän kuukauden jälkeen. Vaikutukset olivat kooltaan pieniä neljän kuukauden seurantamittauksessa (d = 0,20–0,23, NNT (NumberNeeded-to-Treat) = 7,69–8,93).
Kokonaisarvio: ICDP – Kannustava vuorovaikutus -ohjelmalla on kohtalainen dokumentoitu näyttö (4/5) suomalaisessa kontekstissa. Ohjelman implementointivalmius toteutui kuudessa osa-alueessa kymmenestä.
