dc.description.abstractTulehduksellisten suolistosairauksien määrä kasvaa jatkuvasti. Sairauden seurannalla on keskeinen rooli hoidossa. Seurantaa kehitetään systemaattisemmaksi siten, että potilaan rooli korostuu sairauden omatoimisessa seurannassa. Systemaattisen ja omatoimisen sairauden seurannan on tarkoitus helpottaa potilaan arkea ja mahdollistaa terveydenhuollon resurssien keskittämisen potilaisiin, joiden oireet ovat vaikeita. Potilaiden näkemys sairauden omatoimisesta seuraamisesta on tärkeä erityisesti silloin, kun sairauden seurantaa kehitetään suuntaan, jossa potilaan rooli on aktiivinen. Tässä pro gradu -tutkimuksessa selvitettiin kuinka tulehduksellista suolistosairautta sairastavat henkilöt kirjaavat ja seuraavat oireitaan, sekä sitä kuinka he suhtautuvat suunnitteilla olevaan digitaaliseen omaseurantajärjestelmään. Tutkimus toteutettiin Crohn ja Colitis yhdistyksen jäsenille kyselytutkimuksena. Tutkimusaineisto koostui 1430 vastauksesta. Aineisto analysoitiin frekvenssianalyysin, ristiintaulukoinnin ja logistisen regressioanalyysin avulla, ja tilastollinen merkitsevyys testattiin X² -riippuvuustestillä. Tulokset osoittivat, että suurin osa kyselyyn vastanneista ei kirjannut sairautensa oireita itsenäisesti. Useimmat suhtautuivat kuitenkin myönteisesti suunnitteilla olevaan omaseurantajärjestelmään, jossa oireiden kirjaus on systemaattista. Järjestelmän tärkeimpänä ominaisuutena pidettiin tietojen siirtymistä automaattisesti terveydenhuoltoon. Osaa huolestutti se, että digitaalisen omaseurantajärjestelmän myötä perinteinen yhteydenotto hoitotiimiin voisi hankaloitua. Potilaiden omatoimiseen sairauden seurantaan perustuvalle järjestelmälle näyttäisi olevan tarvetta, ja sen käyttöön halukkuutta potilaiden keskuudessa. Systemaattisen seurannan merkitystä potilaille tulisi korostaa, jotta he motivoituvat kirjaamaan sairautensa oireita säännöllisesti. Sairauden seurantatietojen välittäminen terveydenhuoltoon on tärkeää potilaille, joten omaseurantajärjestelmän ja terveydenhuollon hoitoyksikön välillä tulee olla yhteys. Potilaiden mahdollinen huoli siitä, ettei hoitotiimiin saisi yhteyttä omaseurantajärjestelmän käytön myötä, tulee huomioida kerrottaessa järjestelmästä potilaille. Saatujen tulosten perusteella omaseurantajärjestelmää voidaan rakentaa potilaiden näkökulma huomioiden. Tutkimuksen tulokset antavat aihetta lisätutkimukseen potilaiden näkemyksestä myös potilasjärjestön ulkopuolella.fi
dc.description.abstractThe number of inflammatory bowel diseases is constantly increasing. Monitoring the disease plays the key role in the treatment. The aim is to develop the monitoring system more systematically so that the role of the patient is emphasized in the self-monitoring of the disease and the transmission of information to health care. This facilitates both the daily life of the patient and enables the resources of health care to focus in patients whose symptoms are difficult. Patients vision of self-monitoring is particularly important when monitoring will be developed to the direction where role of the patient is active. This master thesis investigated how individuals with inflammatory bowel disease record and follow their symptoms. Second, their attitude to the planned digital self-monitoring system was studied. The study was conducted as a questionnaire for Crohn's and Colitis association members. In addition, the questionnaire could also be answered at the association's Facebook and home page, by people not belonging to the association. The research material consisted of 1430 responses. The material was analyzed by frequency analysis, cross table and logistic regression analysis, and the statistical significance was tested by the X² dependency test. The results showed that most respondents did not record the symptoms of their illness independently. However, for the most part responded positively the planned self-monitoring system, where the symptom registration is systematic. According to the respondents, the most important feature of the system was the automatic transfer of data to healthcare. Some respondents were concerned that the use of digital self-monitoring system could cause difficulties to get contact with the healthcare team by traditional way. There seems to be a need and willingness among patients for use the self-monitoring system. The importance of systematic monitoring for patients should be emphasized in order to motivate them to record the symptoms of their illness regularly. Transmission of disease monitoring information to health care is important for patients, so the self-monitoring system should have feature of data mediation which should be communicated clearly to patients. Patients’ possible concern over the lack of contact with the care team when using the self-monitoring system should be considered when recommending use of the system to the patients. Based on the results obtained, the self-monitoring system can be built with the perspective of the patients. This study gives a reason for additional study of patients view outside of the patient organization.en
