| dc.description.abstract | Tutkimuksessa selvitettiin yhteistyökatkosten esiintymistä ja ilmenemistä yhden psykoterapian aikana. Selvitimme mikä johti katkoksiin, mitä katkostilanteissa tapahtui, miten terapeutti toimi katkostilanteissa ja missä määrin katkokset ratkesivat. Tutkimuksemme on toteutettu sisällönanalyysinä, joka pohjaa Safranin ja Muranin katkosteorioihin. Katkosten ja ratkaisuyritysten havaitsemisen välineenä käytimme 3RS-manuaalia.
Katkosmarkkerit olivat hajaantuneet epätasaisesti koko terapian ajalle ja tyypillisimpiä olivat vetäytymiskatkokset. Ratkaisuyrityksiä esiintyi todennäköisemmin kohdissa, joissa oli useampi katkosmarkkeri. Tutkimuksessa nousi esiin kaksi istuntoa, joiden katkokset osoittautuivat erityisen haitallisiksi allianssin kannalta. Ensimmäisessä käsiteltiin luottamusta yhteisyösuhteessa ja toisessa pettymystä. Katkoksiin johti asiakkaan tyytymättömyys terapeutin toimintaan sekä pettymys terapiassa etenemiseen. Asiakas ilmaisi tyytymättömyyttään epäsuorasti esimerkiksi kätkien kritiikkinsä kertomuksiin arjestaan. Terapeutilla oli haasteita havaita katkostilanteita ja toimia niiden aikana rakentavasti, mikä ilmeni defensiivisyytenä, passiivisuutena, asiakasta vastuuttavina kommentteina. Terapeutin itsestä kertominen havaittiin haitalliseksi, kun siihen liittyi terapeutin omien terapiasuhteeseen liittyvien pelkojen paljastaminen ja oman haavoittuvuuden esiintuominen. Terapiassa esiintyneet katkokset eivät ratkenneet, millä oli heikentävä vaikutus allianssiin. Tutkimuksen perusteella terapeuttien koulutuksessa tulisi panostaa yhteistyösuhteen ongelmakohtien havaitsemiseen ja niiden ratkomiseen. | fi |
| dc.description.abstract | This research investigates the occurrence of alliance ruptures during the course of one psychotherapy case. The focus is on discovering the causes for the ruptures and in examining the events that are displayed during the ruptures. The resolution of those events through therapeutic interference and the actions of the therapist are the main interests of this study. The used method is content analysis, based on the rupture theories of Safran and Muran. The ruptures and the resolution strategies were observed with the 3RS-manual.
The rupture markers were scattered unevenly through the course of the therapy. The withdrawal
ruptures appeared most frequently. Resolution attempts were more likely to occur with multiple rupture markers. Within two sessions the ruptures were particularly harmful for the alliance. The first session concerned trust in the therapeutic relationship and the other handled disillusionment. What lead to the ruptures was the discontentment of the client to the actions of the therapist and disappointment to the progress in the treatment. The client expressed her discontentment implicitly for example by hiding her criticism into stories of her everyday life. The therapist had difficulties in detecting the rupture events and acting constructively in their presence, which is manifested in her defensive or passive behavior
and her comments which put the responsibility of the difficulty on the client. The self-disclosure of the therapist was considered counterproductive, when it was associated with the therapist revealing her
own fears concerning the therapeutic relationship and bringing forth her vulnerability. Resolution of the ruptures in the treatment was inadequate, which weakened the alliance between the therapist and the client. According to the study training the therapists to recognize and solve problems in the therapeutic relationship should be given more attention in the education of therapists. | en |
