dc.description.abstractTIIVISTELMÄ Seija Honkalampi (2015). Asiakkaan kokemus alkoholin käytön mini-interventiosta. Terveystieteiden laitos, Jyväskylän yliopisto, (terveyskasvatus ja terveyden edistäminen) pro gradu -tutkielma, 56 s., 5 liitettä. Tämän pro gradu -tutkielman tarkoituksena oli kuvata terveydenhuollon asiakkaan kokemusta alkoholin käytön mini-interventiosta. Mini-interventiolla tarkoitetaan alkoholin käytön puheeksiottamista, terveysneuvontaa alkoholin käytön vähentämiseksi, tietoa alkoholin vaikutuksista terveydelle tai muuta keskustelua alkoholin käytöstä terveydenhuollossa asioinnin yhteydessä. Aineisto kerättiin haastattelemalla seitsemää päihdekuntoutuksessa olevaa henkilöä. Aineiston analyysissä käytettiin Interpretative Phenomenological Analysis (IPA) -menetelmää. Tulosten mukaan terveydenhuollon asiakkaat kokivat mini-intervention tärkeäksi alkoholin aiheuttamien haittojen ennaltaehkäisyssä ja alkoholiriippuvuuden hoidossa. Mini-intervention kokeminen muodostui kahdesta merkityskokonaisuudesta: mini-intervention vuorovaikutuksesta ja asiantuntevasta neuvonnasta. Se herätti toivon paremmasta voinnista, mikä ilmeni helpotuksen ja kiitollisuuden tunteena. Vuorovaikutuksessa aidon välittämisen ja empatian kokeminen vahvisti luottamusta terveydenhuoltoon. Asiakas koki saavansa arvostusta ihmisenä, kun hänen tilanteestaan ja auttamisestaan oltiin kiinnostuneita. Asiantuntevan neuvonnan merkitys mini-interventiossa oli asiakkaan omien voimavarojen vahvistuminen. Voimavarat auttoivat asiakasta ottamaan vastuuta alkoholin käytön vähentämisestä. Asiakkaan elämäntilanteen ja terveydentilan huomioiva neuvonta tuki asiakasta itsenäisyyteen ja vastuunottoon. Pettymystä ja mielipahaa liittyi tilanteisiin, joissa terveydenhuollon asiantuntija ei lähtenyt keskustelemaan alkoholin käytöstä. Tilanteet, joissa terveydenhuollossa asiakkaalle ilmaistiin avun tarjoamisen sijaan paheksuva mielipide alkoholin käytöstä, koettiin huonona kohteluna ja vallankäyttönä. Tilanteet herättivät syyllisyyttä ja häpeää sekä negatiivisia tunteita terveydenhuollon henkilöstöä kohtaan. Tämän tutkimuksen perusteella alkoholin aiheuttamien terveyshaittojen ennaltaehkäisemiseksi ja vähentämiseksi suositellaan mini-intervention systemaattista ja tutkimustietoon perustuvaa toteuttamista perusterveydenhuollossa: Terveydenhuollon henkilöstö reflektoi säännöllisesti alkoholin käyttöön liittyviä asenteita ja normeja pystyäkseen kohtaamaan alkoholia käyttävän asiakkaan arvostavasti ja kunnioittavasti. Tärkeää on alkoholin käytön mini-intervention osaamisen kehittäminen ja asiakkaan voimavarojen aktiivinen tukeminen. Jatkossa on tärkeää tutkia, millaisia ovat terveydenhuollon asiantuntijoiden kokemukset mini-interventiosta, ja millaiset terveydenhuollon organisaation normit ohjaavat mini-intervention toteuttamista. Jatkotutkimusta tarvitaan myös alkoholin käytön mini-intervention vuorovaikutuksen ja neuvonnan vaikuttavuuden arvioimiseksi. dc.description.abstractABSTRACT Seija Honkalampi (2015) Patients’ experiences with an alcohol brief intervention. Faculty of Health Sciences, University of Jyväskylä, (health education and health promotion) Master´s thesis, 56 pp., 5 appendicies. The aim of this Master’s Thesis was to document health care patients’ experiences with an alcohol brief intervention. A brief intervention means bringing up the issue of alcohol use, health counselling for reducing the use of alcohol, information about alcohol’s effects on health, or other discussion about the use of alcohol, as a part of health care services. The data was gathered by interviewing seven persons who were undergoing alcohol rehabilitation. The Interpretative Phenomenological Analysis (IPA) method was used to analyze the data. According to the results, health care patients felt that the brief intervention was important for preventing damages caused by alcohol and in treating alcohol addiction. Experiences with brief intervention could be divided into two large categories: interaction in the brief intervention and competent counselling. For patients, a brief intervention meant an opportunity to succeed in reducing their use of alcohol. It gave them hope of feeling better, which was expressed through feelings of relief and gratitude. Experiencing genuine caring and empathy in interaction strengthened the patients’ trust in health care services. Being interested in the patients’ conditions and helping them made them feel appreciated as patients. Competent counselling as a part of the brief intervention was significant in strengthening the patients’ own resources. These resources helped patients to take responsibility in reducing their use of alcohol. Counselling that took into account the patients’ situations in life and physical health supported them in being independent and taking responsibility. Disappointments and grief were experienced when health care experts did not initiate discussion about the use alcohol. Situations, where instead of offering help, patients were given a disapproving opinion on the use of alcohol, were seen as bad treatment and oppressive. These situations led to feelings of guilt and shame, as well as negative feeling towards health care personnel. Based on this study, in order to prevent and reduce health problems caused by alcohol, carrying out systematic brief interventions based on researched information is recommended as a part of basic health care services: health care personnel regularly reflect on their attitudes and norms regarding the use of alcohol in order to appreciatively and respectfully face a patient who uses alcohol. And it is importat that health care personnel develop their expertise in alcohol brief interventions and actively support patients’ resources for controlling their use of alcohol. In the future, it is important to study what kind of experiences health care personnel have had with brief interventions and what kind of organizational norms guide the use of brief interventions as a part of health care services. Further research is also required to assess the effectiveness of interaction and counselling in alcohol use brief interventions.
