dc.description.abstractTämän tutkimuksen tarkoituksena oli tutkia Pirkanmaan Hoitokodissa vuosina 2000−2013 pidettyjen musiikkiterapiasururyhmien asiakkaita, musiikkiterapian menetelmiä sekä niiden toimivuutta ja keskusteluteemoja sekä luoda musiikkiterapiasururyhmämalli, jota voitaisiin hyödyntää saattohoidossa terveydenhuollon eri sektoreilla. Tavoitteena oli saada saattohoidossa kuolleiden omaisilta tietoa musiikkiterapian käytöstä surutyön tukena. Tutkimus oli retrospektiivinen pitkittäistutkimus, jossa tutkimusaineistona oli Pirkanmaan Hoitokodin musiikkiterapiasururyhmiin vuosina 2000−2013 osallistuneet surevat ja sururyhmistä tehdyt muistiinpanot. Aineiston (N= 94) analysoinnissa käytettiin frekvenssi- ja prosenttijakaumia, ristiintaulukointia sekä laadullista sisällön erittelyä. Musiikkiterapiasururyhmiin osallistuneista surevista 83 % oli naisia ja lähes 70 % yli 56 -vuotiaita. Valtaosa surevista oli menettänyt puolisonsa (80 %). Yleisimpiä musiikkiterapian menetelmiä sururyhmissä olivat musiikkimaalaus, kirjoittaminen ja laulujen kuuntelu. Keskeisimmät keskusteluteemat olivat sairastuminen, kuolema, erilaiset surun tuntemukset, muistot ja surusta selviytyminen. Teemojen suhteen musiikkiterapiasururyhmät olivat hyvin yhdenmukaisia, lukuun ottamatta uskoon liittyvän teemaa, jonka esiintymisessä oli havaittavissa huomattavia eroja ryhmien välillä. Musiikista on löydettävissä monenlaisia emotionaalisia merkityksiä ja siksi se toimi erinomaisena peilinä surutyön moninaiseen tunnemaailmaan. Musiikin soidessa ja edetessä mahdollistui myös surevan tunteiden elävöityminen, muuttuminen ja purkautuminen, mutta myös musiikin kokeminen muuttui suruprosessin edetessä. Musiikki edisti surun, usein sanojen ulottumattomissa olevien tunteiden ja mielikuvien esilletuloa. Musiikki herätti muistoja, joiden avulla sureva saattoi muuttaa ja jatkaa kiintymyssuhdettaan menetettyyn läheiseen. Monenlaisten musiikkiterapeuttisten menetelmien avulla suru tuli käsiteltyä symbolisina tuotoksina syvemmin, monipuolisemmin ja rikkaammin kuin pelkästään puhumalla. Lisäksi musiikin kuuntelu toi iloa, lohtua, toivoa ja tarkoituksenmukaisuuden tunnetta, jotka osaltaan auttoivat surevaa jatkamaan elämäänsä. Tutkimus antoi arvokasta tietoa musiikkiterapian mahdollisuuksista tukea surevaa omaista läheisen kuoleman jälkeen. Edelleen tarvitaan lisää tutkimusta musiikkiterapian vaikuttavuudesta saattohoidon alueella Suomessa.fi
dc.description.abstractThe goal of this thesis is to examine the clients in the music therapy bereavement group sessions held at the Pirkanmaa Hospice from 2000 to 2013, the methods used in these music therapy sessions and the topics of discussion. Moreover, during the research a model will be created for the music therapy bereavement group sessions that can be later used in palliative care in different sectors in the field of health care. During this study, information will be gathered about using music therapy as a support in the grieving process for those whose relative died in palliative care. The research was retrospective in its nature and the study material consisted of the experiences of the bereaved and the notes made during music therapy bereavement group sessions in Pirkanmaa Hospice from 2000 to 2013. In this research different methods, diagrams with distributions according to frequency and percentage, cross tabulation and also qualitative analysis of content was used in order to get a complex analysis of the phenomenon. Among the bereaved who participated in the music therapy group sessions 83 % were women and almost 70 % were over 56 years old. The majority (80 %) of the participants had lost their spouse. The methods of music therapy that were used most frequently in the group sessions were painting, writing and singing songs. The most important topics of discussion were getting ill, death, different emotions in grieving, memories and how to cope with the grief. The different topics were discussed on the same extent within all the groups. The only exception was the issues of spirituality, which were discussed significantly more in some groups than in others. Music brings up different kinds of emotions and it is a source for creating emotional meanings. In this way it functions as a mirror which reflects the variety of the emotions that a grieving person might have. While listening to music the bereaved could feel their emotions change and release. The way to experience the music varies during the grieving process. The music helped to bring up the grief and other feelings and images far beyond the words. Music arose memories which helped the bereaved to form a new kind of relationship to the deceased. The variety of different methods, that music therapy offers, made it possible for the bereaved to process the grief on a deeper and richer level than by simply talking about their feelings. Moreover, listening to music brought joy, consolation, hope and feelings of meaningfulness in the life of the bereaved. This, in turn, helped the bereaved to continue their lives. This thesis gave valuable information about the use of music therapy in a bereavement group. However, more research of the impact of music therapy is needed in palliative care in Finland.en
