dc.description.abstractTausta: Päänsärky on yksi yleisimmistä neurologisista oireista, sillä lähes kaikilla on jossain vaiheessa elämäänsä päänsärkyä. Tämän vuoksi on tärkeää selvittää myös ei-lääkkeellisten hoitomuotojen käyttöä päänsäryn hoidossa. Tämän systemaattisen katsauksen ja meta-analyysin tarkoituksena oli tutkia terapeuttisen harjoittelun vaikutusta päänsärkyyn. Tutkimusmenetelmä: Tähän analyysin haettiin satunnaistettuja, kontrolloituja tutkimuksia seuraavista tietokannoista vuosilta 1980 - 2013 syyskuu; PubMed (Medline), The Cochrane Central Register of controlled Trials, SPORTDiscus, Cinahl ja The Physiotherapy evidence based database (PEDro). Katsaukseen hyväksyttiin tutkimukset, jotka käsittelivät terapeuttisen harjoittelun interventioita päänsäryn hoidossa. Terapeuttisen harjoittelun interventioita käsiteltiin päänsäryn pääluokituksien mukaisesti. Primaariset päänsärky tutkimukset koskivat migreeniä ja tensiopäänsärkyä, sekundääriset päänsärkytutkimukset liittyivät kaularankaperäiseen päänsärkyyn. Lisäksi tutkimuksia jaettiin myös luokittelemattomiin päänsärkytutkimuksiin, joissa tutkittavia ei jaettu yllämainittuihin päänsäryn luokkiin.Tutkimusten laatu arvioitiin 12 laatuarviointi asteikolla. Meta-analyysit suoritettiin The Cochrane Collaboration Review Manager 5.2 – ohjelmalla. Tulokset: Katsaukseen hyväksyttiin sisäänotto- ja poissulkukriteerien perusteella 15 tutkimusta. Tutkimusten perusteella, terapeuttisen harjoittelun interventioilla ei ilmeisesti ole vaikutusta kaularankaperäisessä päänsäryssä (p=0.30) ja migreenissä (p=0.15) kun vertaillaan kontrollihoitoihin kivun intensiteetin osalta. Terapeuttisella harjoittelulla ei liene vaikutusta tensiopäänsäryn (p=0.95) ja luokittelemattoman päänsäryn kivun intensiteettiin (p=0.35). Johtopäätökset: Tutkimusten mukaan kaularankaperäisen päänsäryn ja migreeniin liityvät interventiot eivät ilmeisesti vähennä päänsäryn intensiteettiä (Näytön aste B). Tension-eli jännityspäänsärkyyn ja luokittelemattomaan päänsärkyyn liittyvillä terapeuttisen harjoittelun interventioilla ei liene vaikutusta päänsäryn intensiteettiin (Näytön aste C). Tutkimustuloksiin on myös suhtauduttava varauksella tutkimusten heterogeenisyyden takia.fi
dc.description.abstractBackground: Majority of the population in worldwide suffers headache during their lifespan. Therefore, it is reasonable to find out a non-pharmacological therapies to treat headache. The purpose of this systematic review and meta-analysis was to investigate the effect of therapeutic exercise on headache in adults. Methodology: For this systematic review and meta-analysis a broad search was performed of 5 databases for the years 1980 - 2013 September as follows: PubMed ( Medline ) , The Cochrane Central Register of Controlled Trials , SPORTDiscus , Cinahl and The Physiotherapy evidence-based database (Pedro ). Randomized controlled studies (RCT) that examined therapeutic exercise intervention in the treatment of headaches were included in the analysis. Therapeutic exercise intervention studies related to primary headache classification migraine headaches and tension-type headache, and secondary headache classification included cervicogenic headache. In addition, studies were also included unclassified headache class in which the subjects was not divided into the above-mentioned categories of headaches. Analysis of studies conducted by the quality of the evaluation and the results independently. The meta-analyzes were carried out by the Cochrane Collaboration Review Manager 5.2 - software. Results: In total 15 RCTs fulfilled the inclusion criteria and were included in the synthesis and meta-analysis.Therapeutic exercise interventions do not appear to have effect on cervicogenic headache (p=0.30) and migraine (p=0.15) when compare treatments for control group in pain intensity. Also exercise therapy do not have effect on tensiontype headache (0.95) and unclassified headache (0.35). Conclusions: Studies have shown that cervicogenic headaches and migraine interventions do not appear to reduce the intensity of the headache ( Level of evidence B). Also, tension-type headaches ja unclassified headaches related to therapeutic exercise interventions, may not reduce the intensity of the headache ( Level of evidence C).Study results must be treated with caution because of the heterogeneity of studies.en
