dc.description.abstractMiia Suuriniemi tutki perintötekijöiden vaikutusta murrosikäisten lasten luuston kasvuun ja kehitykseen. Osteoporoosin sukurasitus ei rajoitu koskemaan pelkästään vanhuusiän perinnöllistä luukatoa, vaan vaikutus saattaa ilmetä jo lievinä kasvuhäiriöinä lapsuusiässä. Tietyt perintötekijät saattavat myös altistaa lapsuuden luunmurtumille, jotka omalta osaltaan voivat hidastaa nuoren kasvua, aiheuttaen mahdollisesti jopa pysyviä luustovaurioita.Luusto muodostuu pääosin kollageeniproteiinista, joka antaa luustolle vetolujuutta ja sitkojoustavuutta. Kollageenin rakennetta muuttavat geneettiset mutaatiot aiheuttavat lähes poikkeuksetta harvinaista, mutta vakavaa synnynnäistä luustosairautta, osteogenesis imperfektaa.Suuriniemi selvitti väestössä varsin yleisten, mutta lievempien kollageenimutaatioiden vaikutuksia terveiden lasten luuston rakenteeseen ja vahvuuteen. Hänen tutkimuksensa osoitti COL1A1-geenin rakennevaihtelun olevan yhteydessä tyttöjen luuston heikkoihin luusto-ominaisuuksiin. Biokemialliset löydökset viittasivat luuston häiriintyneeseen aineenvaihduntaan, mikä saattaa vaikuttaa haitallisesti luuston kasvuun. Tämä saattaa selittää myös aikaisemmissa tutkimuksissa tehtyjä havaintoja kyseisen rakennevaihtelun ja vanhuusiän luunmurtumien välisistä yhteyksistä. Suuriniemi löysi merkittävän yhteyden myös COL1A2-geenin rakennevaihtelun ja lasten luunmurtumien välillä.Lapsen murrosiän kehitystä ja luuston kasvua säätelee voimakkaasti estrogeeni. Katekoli-O-metyylitransferaasi (COMT) on entsyymi, joka hajottaa tätä hormonia. Suuriniemen tutkimus osoitti COMT-geenin rakennevaihtelun vaikuttavan veren estrogeenipitoisuuteen ja aikaistavan murrosiän puhkeamista. Kyseisellä rakennevaihtelulla voi olla merkittäviä vaikutuksia esimerkiksi aikuisiän hormonikorvaushoidon onnistumiseen. dc.description.abstractGiven the current trends of aging population and the growing adoption of unhealthy and sedentary life styles among children and adolescents, bone fragility with associated fractures will truly become a global problem in years to come. Even though the foundation for skeletal health is established during young ages and is strongly governed by the heredity, our current understanding of the genetic basis of paediatric bone mass and growth process is limited. Therefore, the purpose of this work was to elucidate this issue by investigating four important bone-related polymorphic genes in relation to several bone phenotypes and risk of fragility fractures in healthy teenage schoolgirls. Concerning the genes related to the organic matrix of bone, polymorphism of the type I collagen α1 gene (COL1A1) was found to associate with overall poor quantity as well as quality of bone, while that of the type I collagen α2 gene (COL1A2) contributed to increased risk of fractures independently of bone density. Regarding the genes related to the action and metabolism of estrogen, polymorphism of the estrogen receptor α gene (ER-α) was found to modulate the effect of exercise on bone density, and that of the catechol-O-methyltransferase gene (COMT) associated with an early sexual and musculoskeletal development, via elevated concentration of estrogen. Together, these findings provide new information on the genetic basis of children’s bone growth, which will be useful considering the future challenge of early identification of children at risk for bone fragility and prevention of osteoporosis before a fracture occursen
