dc.description.abstractTausta: Tasapainon ongelmat ovat yleisiä aivoverenkiertöhäiriön (AVH) sairastamisen jälkeen. Vaikka AVH-kuntoutujien kuntoutusmenetelmät ovat kiinnostuksen kohteena, on tasapainoharjoittelun vaikuttavuudesta niukasti näyttöön perustuvaa tietoa. Tarkoitus: Tutkia AVH-kuntoutujien tasapainoharjoittelun vaikuttavuutta tasapainoon tasapaino- ja kävelymuuttujilla RCT-tutkimusten perusteella. Aineisto ja menetelmät: Systemaattinen kirjallisuushaku tehtiin Ovid Medline, CINAHL, Embase, Physiotherapy Evidence Database, the Cochrane Central Register of Controlled Trials sähköisistä tietokannoista sekä käsihaulla. RCT-tutkimukset, joissa koehenkilöiden ikä oli yli 18- vuotta ja jotka oli julkaistu englannin, suomen, saksan tai ruotsin kielellä otettiin mukaan tutkimukseen. Haut tehtiin tammikuun 2012 loppuun. Meta-analyysi suoritettiin kolmella eri ryhmäjaolla: 1) tasapainoharjoittelu verrattuna ei/lume harjoittelu, 2) tasapainoharjoittelu verrattuna muu harjoittelu ja 3) tasapainoharjoittelu perinteisen fysioterapian lisänä verrattuna perinteinen fysioterapia. Näytön aste ilmaistiin tutkimusten määrän ja laadun perusteella; tasokas, kohtalainen, heikko tai ei näyttöä. Tulokset: 29 RCT-tutkimusta valikoitui mukaan tutkimukseen. Tasapainoharjoittelu paransi tasapainomuuttujien tuloksia (vahva näytön aste), mutta ei kävelytuloksia (vahva näytön aste), verrattuna ei/lumeharjoitteluun. Tasapainoharjoittelu ei parantanut tasapaino (kohtalainen näytön aste) tai kävelymuuttujien tuloksia (kohtalainen näytön aste) verrattuna muuhun harjoitteluun, eikä tasapainomuuttujien tuloksia (vahva näytön aste), kun tasapainoharjoittelua tutkittiin perinteisen fysioterapian lisänä ja verrattiin perinteiseen fysioterapiaan. Yhteenveto: AVH-kuntoutujien tasapainoharjoittelu kehittää tasapainoa paremmin kuin ei/lumeharjoittelu. Aktiivinen harjoittelu, joka kehittää vartalon ja alaraajojen toiminnallisuutta kehittää tasapainoa ja kävelyä yhtä paljon kuin erityinen tasapainoharjoittelu. Tasapainoharjoittelu fysioterapian lisänä ei ole tehokkaampaa kuin perinteinen fysioterapia. Lisätutkimuksia tarvitaan AVH-kuntoutujien tasapainoharjoittelun ja –kuntoutuksen kehittämiseksi.fi
dc.description.abstractBackground: Balance deficits are common in individuals after stroke. Although rehabilitation techniques are of interest in the care of stroke patients, there is a lack of evidence-based on effectiveness of balance training. Objective: To determine the evidence of effectiveness of active balance training on the balance and walking outcomes of patients with stroke by reviewing the results of randomized controlled trials (RCTs). Methods: A systematic literature search was conducted in Ovid Medline, CINAHL, Embase, Physiotherapy Evidence Database, the Cochrane Central Register of Controlled Trials and by manual search. RCTs for patients over 18 years old with stroke and published in English, Finnish, Swedish, or German were included in the study. Studies were collected up to January 2012. Meta-analyses were performed separately for three subgroups of studies: 1) Balance training versus no or placebo treatment, 2) balance training versus other treatment and 3) balance training with conventional physiotherapy versus conventional physiotherapy only. The evidence found was rated, according to the number and quality of studies; high, moderate, low or no evidence. Results: 29 RCTs were included in the review. Balance training produced better balance (high evidence), but not walking (high evidence), outcomes compared to no/placebo treatment. Balance training did not improve balance (moderate evidence) or walking outcomes (moderate evidence) when balance training compared to other training or balance outcomes (high evidence) when balance training with conventional physiotherapy was compared to conventional physiotherapy only. Conclusions: Balance training proved to be more effective in improving balance than no treatment or placebo control following stroke. Active training for improving trunk or lower extremities function improved balance and walking as much as specific balance training. Balance training with physiotherapy did not have an extra impact compared to conventional physiotherapy. Further studies are needed to conclude balance training and rehabilitation.en
